Citation Request Form

Please allow two (2) weeks in advance of event.

The office will call to confirm.  Please include precise information.

If an invitation, handout or other printed material is available please e-mail a copy to comptroller.forms@baltimorecity.gov or fax a copy to 410.685.4416.
For EXAMPLE, a retirement, please list company name/department, years of service, etc.; a graduation, list name of school/degree, etc. (A separate/additional page can be emailed or faxed, if necessary)

Attachments larger than 14 MB must be emailed to comptroller@baltimorecity.gov.
Files must be less than 14 MB.
Allowed file types: gif jpg jpeg png bmp eps tif pict psd txt rtf pdf doc docx odt ppt pptx odp xls xlsx ods xml.
